ADDITIONAL INFO TO APPEAR ON ROUTING SLIP.
WOULD LIKE AN AREA FOR MESSAGE OR REMINDER FOR THAT PERSON TO SHOW UP ON ROUTE SLIP THAT WOULD SHOW UP EVERY TIME WITHOUT HAVING TO RE-ENTER MESSAGE. ALSON HAVING SUSCRIBER DOB IN INSURANCE AREA WOULD BE VERY HELPFUL.

additional box for what insurance covers, frequencies or a message/reminder that the office would want to show up on route slips and also add suscribers date of birth to route slip as well
