Need to be able to put different ICD 9 codes per procedure without creating multiple claims
Program currently when posting medical codes, the HCFA claim creates different claims if we do not put the Codes in the same order. We have codes that need to be on the same claim, but they use defferent ICD 9 codes. need this corrected.

With the 2012 CMS form, you are able to add up to 12 dx codes and using the x-code window, you can assign the correct dx code. please let me know if you don't see this.

I have noticed that the ICD 9 codes must apply to each and every line item the same. For example the 520.6 and 528.5 was used when we did D7241(impacted wisdom teeth) and D7960 (frenectomy). OMS makes it mandatory to list 520.6 as an ICD code for a frenectomy! This is unacceptable
