Seperate columns for patient and insurance portions when printing out treatment plan
When printing out the treatment plan for the patient to sign/keep, when choosing to include the UCR (so the patient sees the difference their insurance is making) the insurance estimated payment prints directly on top of the patient estimated portion, with merely a "P" to differentiate which is which. This makes the printed treatment plan messy to look at, and difficult for the patient to understand. What we would like to see is the patient portion and the insurance portion to list separately side-by-side, for ease of understanding, as it does when the UCR is not included in the printed TX plan.
