If a payment creates an adjustment or when creating an adjustment, instead of having the credit sit on the first line of the ledger, create a transaction for the credit to sit on. The transaction would be associated with the date the payment is entered, assigned to the house doctor, on the statemetns it would show up on the date it was created and have a description would be something like 'Credit created' this transaction would age appropriatey based on the date the transaction was created
